National Criminal Background Checks
Effective January 1, 2009 All students currently enrolled in the IU School of Nursing will be required to submit on an annual basis an updated national criminal background check which will be will be conducted through Certified Background. It is the responsibility of each applicant to pay the fee for the screening directly to Certified Background. Each year, students will be required to submit their request for updated an updated national criminal background check report.
Deadline for renewal
Spring admits will be required to submit annually an updated criminal background check by December 15 of the following year.
Fall admits will July 1 be required to submit annually an updated criminal background check by of the following year.
Summer admits will be required to submit annually an updated criminal background recheck by April 15 of the following year.
An updated national criminal background check will also be required for any student being readmitted/transferring to the program.
Instructions for Requesting Background Checks
Upon notification of conditional admission to the IU School of Nursing students should formally request a National Criminal History Background Check be submitted to the IU School of Nursing from the vendor Certified Background. This must be completed within 10 days of being notified of the conditional admission offer. Failure to do so may result in expulsion from the program.

- Newly admitted students should enter the package code number IU34 which will direct your report to be sent to the IU School of Nursing.
Returning students should enter package code IU35RE to complete the annual criminal background recheck.
- Submit payment of $65 for initial state of Indiana check. Additional states, dating back to the past 7 years, can be searched at no additional charge. The cost for annual background check renewal is $30.
- After entering payment information, you will be asked to provide your personal identification information and data for each required search included in the package.
- Please review all your information and make changes before submission. You should then print your order information page. You will receive an e-mail confirmation when your order is complete which contains your password and instructions for retrieving your results.
