Fees and Costs
Cost: $175
The cost of the program includes the Qualified Medication Aide Basic Curriculum Instructor’s Guide – ISDH October 2003, handouts, breaks, and contact hour record. Lunch is not included.
Payment in full must be received before placement in the class can be guaranteed. Registration for the program is limited to 40 participants. Advance registration is required. A minimum of 10 participants is required for the course to be held. In the event of course cancellation, all registration monies will be refunded. No other costs or fees will be reimbursed because of cancellation.
Payment can be made by check or credit card (MasterCard, Visa, American Express). If necessary, a purchase order can accompany the registration.
Refund Policy
Registration fee less a $60 administrative charge is refundable only if notification is received at least five (5) business days in advance of program.
Transfer Fee
Beginning January 1, 2007, a transfer fee in the amount of 10% of the amount paid will apply when a request to transfer from the currently scheduled program to the next scheduled program is received and approved.
The refund policy will still apply if you later decide to drop the course.
